Foster City– Cal/OSHA has cited two employers for serious accident-related health and safety violations after workers were poisoned by carbon monoxide in an enclosed space at San Francisco International Airport.

Two plumbers from Gladiator Rooter & Plumbing were working in a crawl space on December 22, 2018, replacing underground sewer pipes for airline caterer Gate Gourmet, Inc. Carbon monoxide gas was leaking from the facility, causing one of the workers to lose consciousness. Rescue workers helped the workers, but one of them had to stay in the hospital for two days.

“These workers were lucky because working in confined spaces can be deadly, especially when oxygen levels are reduced or deadly gases are present,” said Debra Lee, deputy chief of enforcement for Cal/OSHA. “Employers must identify and assess potential hazards before workers enter confined spaces so they can ensure workers are trained and a rescue plan is in place in the event of an emergency.”

Cal/OSHA’s investigation found that Gate Gourmet, Inc. did not inform Gladiator Rooter & Plumbing that the crawlspace was a permit-required confined space and failed to provide information about the potential hazards of entering of the room arise. Cal/OSHA also found that Gladiator Rooter & Plumbing did not have a safety and health program in place and did not train workers. Additionally, the employer did not develop a confined space program, take no hazard mitigation measures, and have no rescue plan.

Cal/OSHA charged Gladiator Rooter & Plumbing $50,850 for eight violations, including two serious accidental violations, two serious violations, and four general violations.

Major accident-related violations cited included the company’s failure to implement a permit-required confined space program and failure to train its employees to work safely in confined spaces. Major violations cited included the company’s failure to develop and implement a written permit room program and failure to obtain information about hazards to the permit room and make that information available to workers entering the room.

Cal/OSHA cited Gate Gourmet with proposed penalties of $18,000 for a serious accident-related violation for failing to communicate with Gladiator Rooter & Plumbing about confined space hazards and precautions.

A violation is classified as serious if there is a realistic likelihood that the actual danger created by the violation could result in death or serious harm, and violations are classified as accidental if the injury, illness or death is caused by the violation is caused.

Confined spaces are spaces that are large enough for worker entry but have limited openings for exit and entry, creating potential atmospheric and spatial hazards. They are found in numerous industries and include water and sewer pipes, boilers, silos, furnaces, vaults, tunnels and pumping stations.

Construction employers should read and follow the confined space guidelines found on pages 26-29 of the updated Cal/OSHA Construction Industry Pocket Manual. Cal/OSHA provides additional confined space resources to general industry employers.

All employers in California are required to have an effective written injury and illness prevention program, a safety program designed to identify, evaluate and control workplace hazards. Cal/OSHA has online tools and publications to help employers establish an effective safety program.

Cal/OSHA helps protect workers from workplace health and safety risks in nearly every workplace in California. SAN FRANCISCO (CBS SF/BCN) – Suspects attempted to rob a 40-year-old man in San Francisco’s South of Market neighborhood on Sunday afternoon, and ended up shooting him, resulting in life-threatening injuries, police said Tuesday.

The shooting was reported at 3:48 pm Sunday in the area of ​​Mary and Natoma streets, where the victim was walking when two suspects got out of a vehicle, both pointed firearms at him and demanded his property, according to police.

A struggle ensued, during which one of the suspects shot the victim. The pair fled in the vehicle with his property and remained at large as of Tuesday, police said.

SAN FRANCISCO (CBS SF) – San Francisco police have reopened an area of ​​Dolores Street in Mission Dolores Park that was closed Monday morning over a suspicious device, according to authorities.

Police said officers responded at about 10:12 a.m. in the 600 block of Dolores Street near Cumberland Street about a suspicious device. Arriving officers set up an area at the crime scene that blocked off vehicle and pedestrian traffic in the area while they conducted the investigation.

TaskRabbit, an online marketplace for freelance workers owned by Swedish furniture chain IKEA, announced that it had been attacked by cyber criminals.

On Monday, the San Francisco company deleted its website and app, urging customers to change their passwords as it was investigating a “cybersecurity incident”. The company said it was working with a cybersecurity firm but did not disclose what happened, how many people may have been affected, or how it learned about the breach.

The company said it had notified customers and “taskers,” those offering their services and being reviewed by the company, and said it would reschedule any unfinished tasks and compensate workers for the tasks that were done on Monday due to the disruption could not be completed.

A company representative did not respond to a request for additional information on Monday afternoon.

Antonio Brown has been suspended for the first eight games of the 2020 season due to multiple off-field incidents and the veteran recipient is now facing a civil lawsuit regarding one of them.

According to Broward County court documents Matt Baker received from the Tampa Bay Times, Brown was sued by a driving truck driver who was allegedly assaulted by Brown and Brown’s coaches earlier last year. The driver, Anton Tumanov, claims in the lawsuit that he brought Brown’s belongings from California to Hollywood, Florida, last year when he “encountered resistance and violence”.

Tumanov said Brown and Brown’s trainer Glenn Holt attacked his vehicle and broke the key before “verbally and physically assaulting him and causing serious personal injury”. The driver says that as a result of the incident, he “continues to suffer serious physical injuries, financial losses and emotional distress”.

Holt and Brown face assault and battery charges. Tumanov is seeking more than $ 30,000 in damages.

Police were called to Brown’s Hollywood home in January 2020 after he was accused of assaulting a driving truck driver over a payment problem. More details about the incident can be found here.

Brown was arrested not long after the incident and did not advocate a criminal battery charge last June. The 32-year-old avoided jail time and received two years probation, 100 hours of community service and a psychological examination.

Brown is unlikely to receive further discipline from the NFL, as the incident with the moving truck was part of the reason he was suspended last season.

Brown caught 45 passes for 483 yards and four touchdowns with the Bucs last season. He recently signed a one-year contract with the team.
