The San Francisco VA Health Care System (SFVAHCS) offers residents the opportunity to practice in innovative and progressive pharmacy services. Our long-term Postgraduate Year 1 (PGY1) and Postgraduate Year 2 (PGY2) pharmacy residency programs combine aspects of clinical pharmacy services, teaching, administration, research, pharmacoeconomics and operations.

PGY1 Pharmacy Residency Program

The Pharmacy Service offers three PGY1 pharmacy residences:

• PGY1 pharmacy residence
• PGY1 Pharmacy Residency with geriatric electives
• PGY1 Pharmacy residency with home care electives

The PGY1 pharmacy residence (NMS code 188313)

The PGY1 Pharmacy Residency offers a flexible program that offers a focused clinical, teaching, and administrative experience with experienced, individualized guidance. Required clinical rotations include acute care (internal medicine, critical care, or surgical specialty) and ambulatory care. The resident will also gain experience in other required learning experiences such as: B.: Introduction to drug safety; inpatient surgeries; Form management, teaching certificate and main residence/pharmacy management. Elective rotations are available for infectious diseases; inpatient anticoagulation; pain therapy; Oncology, Advanced Internal Medicine, Drug Safety, Pharmacy Administration and Geriatrics/Long Term Care. Residents may prescribe UCSF and UOP IPPE and APPE students. Other responsibilities include: attending emergency medical care (code blue and acute stroke); and Completion of a quality improvement or research project presented at the Western States Conference for Pharmacy Residents, Fellows and Preceptors. All PGY1 residents actively participate in the EdPACT (Education in Patient Aligned Care Team) program (EdPACT), which promotes a patient-centered, interprofessional team environment.

The PGY1 pharmacy residence with geriatric electives (NMS code 188322)

One position is reserved each year for residents who desire more experience in geriatrics. Residents who choose geriatric electives complete the same core rotations as other PGY1 pharmacy residents in the San Francisco VA Health Care System, but also complete 12-week geriatric rotations in place of other electives. Applicants must apply separately for this program using the NMS code listed above.

Possible geriatric rotations include:

The PGY1 Pharmacy Residence with Ambulatory Care Electives (NMS Code 188321)

Two positions are reserved each year for residents who desire more home care experience. Residents who choose outpatient care electives complete the same required rotations as other PGY1 pharmacy residents in the San Francisco VA Health Care System, but also complete a 12-week longitudinal experience in additional outpatient care clinic rotations in lieu of other electives. Applicants must apply separately for this program using the NMS code listed above.

PGY2 Pain Management and Palliative Care Residency Program

program purpose

Postgraduate year two (PGY2) pharmacy residency programs build upon the Doctor of Pharmacy (Pharm.D.) training and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies offer residents the opportunity to function independently as practitioners, conceptualizing and integrating accumulated experience and knowledge, and incorporating both into the delivery of patient care or other advanced practice environments. Residents who successfully complete an accredited PGY2 pharmacy residency will be prepared for advanced patient care, academic, or other specialized positions, along with board certification when available.

Program Goals and Objectives

1. Develop clinical skills needed for safe and effective pain management

2. Demonstrate effective oral and written communication skills

3. Effectively integrate with interdisciplinary teams to influence positive outcomes in patient care

4. Develop leadership and management skills

5. Provide training for patients, nurses and other health professionals

6. Conduct a quality improvement study and/or medication use assessment to improve medication use processes

The PGY2 Pain Management and Palliative Care Pharmacy Residency at the San Francisco VA Health Care System is a year-long residency designed to prepare a pharmacist to practice as a Clinical Pharmacy Specialist in pain management and/or palliative care. The program is designed in accordance with the PGY2 Pain Management & Palliative Care Pharmacy Residencies competency areas, goals and objectives required by the American Society of Health-System Pharmacist (ASHP). Direct patient care experiences, case-based application, and choice experiences are designed to engage the resident in a variety of content areas: acute pain management, chronic pain management, psychiatric conditions, palliative care, hospice, interventional medicine, and integrative medicine.

SAN JOSE (CBS SF) – A car that was plowed outside a CVS pharmacy in San Jose on Wednesday.

The San Jose Fire Department said the crash happened around 12:11 p.m. at the CVS store on Blossom Hill Road in South San Jose.
